1/7/2023 EnchantaVaughn Approved Behavior: Mom and client welcomed QMHP back into the home for a 2-hour visit. Client presented with a normal mood and affect. Client displayed calm and peaceful expressions. Client was reported to be having a good evening by his mother. Client reported that he is feeling good today and was ready to face his challenges. Client wanted to show his mom that he was handling himself well without depending on extra help. Client stated that having a counselor kept him on his toes. Client displayed good interaction and contributing his thoughts to the therapeutic discussion. Client displayed good eye contact. Client did not talk over QMHP. Client exhibited an appropriate attitude, due to his cooperation and communicating well during the session. Behavior: Client was in a pleasant mood today and presented good behaviors, as evidence of him smiling and being polite toward his family. Client was seen interacting and engaging appropriately due to being sociable and respectful to his family. Client has implemented interventions and strategies taught by QMHP in his daily activities. Client shows understanding of simple direction and follows through effectively. Client displays a great attitude and high level of participation at this time, which is supported and encouraged by VACCD. Client appears to be calm and relaxed after today’s intervention. Client was applauded for his teamwork skills and presenting excellent behaviors. Intervention: QMHP engaged in conversation with client about his day. The QMHP demonstrated and provided positive and empathetic acceptance with utilizing talk therapy and active listening skills. QMHP gave client the opportunity to express and explore his perceptions of how his symptoms of ADHD can affect him. QMHP educated client about what he can do to make being at home fun and interesting, such as reading his favorite book, drawing in a journal, or laying down and closing his eyes and practicing the mindful behaviors. QMHP reviewed the positive things that client had done and encouraged him to continue in making positive decisions. Intervention: QMHP observed client’s behaviors as seemingly calm and controlled as demonstrated by his ability to release his energy appropriately in a social environment without instructions. QMHP introduced the therapy activity. The activity’s objective is to increase self-esteem by receiving positive comments from others. The activity was also completed to hear positive comments about themselves. QMHP will provide mediation and relaxation techniques, as well as, simple praises and encouragement to continue the positive behaviors he was exhibiting. QMHP modeled pro-social behaviors for client to strengthen his understanding of what to do in school, at home, and in the community. Response: Client discussed mental illness and its causes as a family. Client displayed interest in learning about ADHD. Client acknowledges expectations and reviews goals and objectives with counselor and parent. Client was able to reevaluate himself and display a more positive attitude throughout the session. Client is reported building healthy relationships in the home. Client shared some of the things he does at home to help him relax. Client confirmed that he still write beats as a way to take his mind from a stressful state to a more peaceful state. Client showed good understanding of what it means to be respectful, encouraging, mindful, and fair to others. Response: Client was receptive and open to the interventions, as he was attentive and focused on the information and activities that were being processed without any difficulties. Client responded in a pleasant manner acknowledging the positive reinforcement that the QMHP provided. Client has become more involved in age appropriate social activities and was given positive reinforcement for these choices. Both client and guardian provided feedback, actively participated, and engaged with QMHP. Client communicated that he felt safe and comfortable talking to the in-home counselor. Client realized that he has made progress and is starting to express more passion for counseling and being at home.
